Before you attend any of my courses it is important that you complete the course application form which must have a parental or guardian's signature, you can download the form here. Second and importantly is the emergency contact form which must inform me of any allergies, medication or conditions of the student attending, again these must be signed by a parent or legal guardian you can download the form here.
